POSITION TITLE: Transition Coordinator (ESS)                         

REPORTS TO: ESS Director

DEPARTMENT: Exceptional Student Services                            

CATEGORY: Certified

CONTRACT TERM: Ten Months (10)                                           

CLASSIFICATION: Exempt

 

JOB SUMMARY:  ESS Transition Coordinator who is highly qualified, innovative, enthusiastic and looking for the opportunity to prepare students for a successful future.  Hopi Jr. Sr. High School (HJSHS) is committed to helping all students develop skills in the areas of personal-social growth, educational planning and career and vocational development.  The Transition Coordinator assists students with disabilities and their families in obtaining services necessary to transition from high school to a post-secondary setting, whether for post-secondary education, employment, or long-term support.

 

RESPONSIBILITIES AND DUTIES:

  1. Provides direct instruction to students with disabilities that are designed to meet their individual transition needs.
  2. Develops curriculum, designs and implements lesson plans and assesses student processes.
  3. Disseminates information to staff, students, and parents regarding student needs, progress, and resources.
  4. Coordinates transition activities for college, career, and community readiness.
  5. Manages IEPs for assigned caseload; coordinates and facilitates IEP meetings, writes IEPs, and communicates with general education teachers and support staff.
  6. Facilitates referral, application, and eligibility process for student and families to adult service providers.
  7. Assist with alignment of transition continuum of services at the secondary level.
  8. Performs other duties as assigned.

 

KNOWLEDGE AND SKILLS REQUIRED:

  1. Understanding of disability areas under IDEA.
  2. Knowledge of how disabilities impact an individual’s ability to function in the post-secondary setting.
  3. Knowledge of state and federal laws/regulations that protect individuals with disabilities in education, employment and community.
  4. Knowledge of state and district policies and procedures under IDEA.
  5. Knowledge of adult service providers and services within each.
  6. Skill in establishing and maintaining effective working relationships with others.

 

QUALIFICATIONS:

  1. Bachelors or Master’s degree in Exceptional Student Services education including specific courses in transitions.
  2. Minimum of three years of teaching experience.
  3. ADE Teaching Certification is required with an Exceptional Student Services endorsement.

 

PREFERRED QUAILIFICATIONS:

  1. Hopi/Indian Preference in employment considered.
  2. Valid State Driver Licenses.
  3. Valid Arizona Department of Public Safety Fingerprint Clearance Card.
  4. Must successfully pass employment background checks within the last five years, which include state, federal and local tribe.
  5. First Aid/CPR card.
